So I’ve placed the required local Board of Health forms right here for you to download, complete, and submit.
Using these forms is mandatory — but finding them no longer has to be a scavenger hunt.

Middletown Twp. Cottage Food Permit Holders Only
For markets within Middletown Township, NJ only. Email all to bamodeo@middletownnj.org and CC health@hearhereevents.com 14 days before the event.
Must include all supporting documents. All fees are the responsibility of the permit holder.
