Board of Health Forms

Food Vendors: All food vendors must submit the required Board of Health paperwork for each event, including all supporting documents (satisfactory inspection certificates or cottage permits). All permit and application fees are the responsibility of the permit holder. If Hear Here does not have a copy of your complete paperwork, you will not be permitted to vend at the event. All certificates must be displayed at the event.
