Food Vendors: Let Me Make Your Life Easier

Paperwork isn’t fun. Health Department paperwork is less fun.

So I’ve placed the required local Board of Health forms right here for you to download, complete, and submit.
Using these forms is mandatory — but finding them no longer has to be a scavenger hunt.

If you’d rather not touch paperwork at all, I offer a $50 “I’ll Do It For You” option where I complete and submit everything on your behalf. Just send me an email requesting this service and we’ll get your business squared with the powers that be.

You focus on cooking. I’ll handle the forms and the fine print.

Middletown Twp. Cottage Food Permit Holders Only

For markets within Middletown Township, NJ only. Email all to bamodeo@middletownnj.org and CC health@hearhereevents.com 14 days before the event.

Must include all supporting documents. All fees are the responsibility of the permit holder.

Middletown Twp. Temp. Food Establishment Permit

For markets within Middletown Township, NJ only. Email all to bamodeo@middletownnj.org and CC health@hearhereevents.com 14 days before the event.

Must include all supporting documents. All fees are the responsibility of the permit holder.

Jersey Shore Regional Health Commission

For markets and events within the Borough of Allenhurst, NJ only. Email all to dastino@jsrhc.org and cc to health@hearhereevents.com at least 14 days before the event.

Must include all supporting documents. All fees are the responsibility of the permit holder.

Monmouth County Board of Health

Email all to health@hearhereevents.com at least 14 days before the event.

Must include all supporting documents. All fees are the responsibility of the permit holder.

Ocean County Board of Health

For The Market at Adventure Crossing. Email all to Jprotonentis@ochd.org and CC’s to health@hearhereevents.com at least 14 days before the event.

Must include all supporting documents. All fees are the responsibility of the permit holder.

Howell Special Events Packet

For Howell Farmers Market and Tall Oaks Farm + Makers Markets. Email all directly to Angela at AMartino@twp.howell.nj.us and cc health@hearhereevents.com at least 14 days before the event. Must display certificate at event.

Must include all supporting documents. All fees are the responsibility of the permit holder.

Keyport Board of Health

For Town & Country Inn events and Markets. Email all directly to dpurcell@keyportonline.com and CC’d to health@hearhereevents.com at least 14 days before the event.

Must include all supporting documents. All fees ($15 check to Borough of Keyport) are the responsibility of the permit holder.

North Brunswick Board of Health

For North Brunswick events and Markets. Email all directly to health_inspector@northbrunswicknj.gov‍ ‍and CC’d to health@hearhereevents.com at least 14 days before the event.

Must include all supporting documents. All fees are the responsibility of the permit holder.

Parsippany Board of Health

There is no fee for this paperwork for 10×10 vendors. Please include health@hearhereevents.com on the list of approval email so I know you’re approved.

Food Trucks may incur a separate fee. Please remember to contact the Fire Bureau for their inspection procedures- see below

Wall Twp.Temp. Food Establishment Permit

For markets within Wall Township, NJ only. Forms are to be submitted electronically 14 days before the event.
